Assessment task:
choose a case studies to develop a care plan to address the needs stated in the care plan. You must use appropriate information from both the taught sessions and you own research.
Students will then need to product a clear care plan that identifies all the needs found and what support would be best suited to help/support/advise the situation. Students should suggest a new service or improvement to a current service and what the the service would attempt to achieve.
